请问申请怀孕和育儿津贴时雇主的工作记录证明怎么开？

老婆打算申请怀孕和育儿津贴，但是材料之一是雇主开具的工作记录证明，可是老板不知道如何开，请知道的朋友告诉一下我。在哪里下载相关表格？谢谢！

this is ROE form from HRSDC, or you could order one from HRSDC

Alberta: call 1 800 561-3992 or complete the ROE order form and fax to 1 800 829-6891

: Y* J9 u/ Y' y& r, F' ^8 vThe person taking your order will ask for:

% @) `5 |- T$ h( H- T1 r7 }% ^your business number employer's number issued by the Canada Revenue Agency (CRA),
the name, telephone number and address of your business.
If you do not have such a number, you will have to request it from CRA before you place an order.  

If you plan on ordering in advance and picking up your ROE forms in person, you will  be asked to provide two pieces of identification, such as valid driver's license, photo identification card, major bank/trust company credit or banking cards.

这是你老板的责任呀？实在不行你让他电话问EI部门吧。
只要有人再他那里工作过并quit过工作，他们就应该开roe给的。无论什么原因她们都要开的。

7 t/ e& d% Y! r% S只知道我老公没quit一份工作，都会有一个roe表格寄到家里的。
