Target:        Low income families with children/youth aged 3-17 residing in Edmonton
Date        May 14, 2005 (Saturday)
4 [6 y0 D- C4 W" {1 c5 l6 K5 MTime        9:30am – 11:30am
Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week

# f$ J  L+ x( ~) v( B; D0 }1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
$ d& N5 a! ?; x; }2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
% S+ v0 y1 {% X+ g" q5 ?  Z* A3. Each child is limited to 1 to 3 programs per organization
1 x8 ], w. ~1 P7 u* n- _4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day

. \4 G1 Z1 A( w: w: s9 wWe will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
