Target:        Low income families with children/youth aged 3-17 residing in Edmonton
3 z: s. I. M, i4 Z# m7 }/ V9 _Date        May 14, 2005 (Saturday)
( e8 Q' F: r) ~Time        9:30am – 11:30am
# u& l' Y. y) }% w" t% q) \Place        ASSIST Community Services Centre (9653-105A Ave)
Programs         Week-long day camps such as swimming, science, games, sports, aquatic activities, treats making, dances, drama, arts and crafts, music etc.
Fees        Ranges from $10 to $35 per week
) L  o) o* k8 J8 `% U7 P! L/ {: b0 x       
8 N) }+ d$ S. Z8 K6 B: e' U6 mRemarks:
1. Must bring along the child’s Alberta Health Care Card, emergency contact number and program fees (cash or cheque only)
/ R  x8 w. G5 h$ V! R) I2. 2004 Tax return / Proof of family income (as program spots are limited, please save for those families who are really in need)
3. Each child is limited to 1 to 3 programs per organization
4. Information on Child’s swimming level when registering for swimming classes
5. Children are not required to be present at the Registration Day
4 Z' G, h* l3 k/ ?6. Registrations will be accepted on a first-come, first-served basis

& ]1 b$ y! ^6 N: M6 K! ^We will also assist families/individual in need to apply for the Fee Reduction Pass on the same day. Adult/children pass holders can enjoy a 75% discount on admission to City operated facilities and regular price registered programs. Please bring along  all your family members’ 2004 tax returns.
